relative path in require_once doesn't work
... this should fix your problem.
So:
require_once(__DIR__.'/../class/user.php');
This will prevent cases where you can run a PHP script from a different folder and therefore the relatives paths will not work.

#define is a preprocessor token: the compiler itself will never see it.
typedef is a compiler token: the preprocessor does not care about it.
You can use one or the other to achieve the same effect, but it's better to use the proper one for your need...
How do I see the extensions loaded by PHP?
It's got to be somewhere in the phpinfo() dump, but I just don't know where. Is it supposed to be under the "Additional Modules" section? Somewhere else? I'm trying to figure out why some extensions don't appear to be loaded, but I don't even know where I should be looking.

Display errors could be turned off in the php.ini or your Apache configuration file.
You can turn it on in the script:
error_reporting(E_ALL);
ini_set('display_errors', '1');
You should see the same messages in the PHP error log.
